Current Patients

  • We do not want missed appointment fees to be an impediment to your care and so we ask for your mutual respect. When you make an appointment we block out our schedule for you and ask that you exercise the same consideration when planning your calendar. As a courtesy to my other patients and to me, please provide adequate notice if you need to cancel or change an appointment.

    • If you are more than 10 minutes late for your appointment, you will have to reschedule and will be charged a late cancellation fee.

    If you do not show up for your appointment and have not canceled the appointment prior to the 24 hour cancellation period we reserve the right to charge the card on file a No-Show Fee
             •        Missed Intake Evaluations: $125
             •        Missed Follow-Up Appointments: $100
    *After the second cancellation within the 24 hour cancellation period, we reserve the right to require payment up front for any future appointments.

    *Any exceptions are to be reviewed by management and will be evaluated on a case-by-case basis.

  • How are medications refilled?

    Medications are typically refilled during your scheduled appointments. At each visit, you will receive enough medication to last until your next appointment.

    What if I need a refill outside of an appointment?

    If you require a refill outside of an appointment, please follow these steps:

    Secure Message: Send your refill request through the patient portal.

    Phone Call: Call the office and leave a detailed message, including your:

    • Full name and date of birth

    • Medication name and dosage

    • Pharmacy name and contact information

    What is the submission process for refill requests?

    • Refill requests must be submitted during business hours.

    • Please allow 48 hours for processing.

    • Requests submitted outside of office hours will be addressed on the next business day.

    What about controlled substances?

    • Controlled substances are only refilled during scheduled appointments.

    • Lost or stolen controlled substance prescriptions will not be replaced unless accompanied by an official police report.

    What if my pharmacy is out of stock?

    Patients are responsible for providing updated pharmacy information. If your pharmacy is out of stock:

    • Notify our office.

    • Provide an alternative pharmacy where the prescription can be redirected.

    If this is your first time logging in to the patient portal, please click on the 'New Sign Up' button to set your initial password. A link will be sent to this email that will allow you to create a password and then sign on to the portal. 

    The portal gives you access to your superbills, assessment scales, patient handouts, treatment plans, and document upload features. You can also use it to request future appointments and explore additional resources.
